Carver County, Minnesota, March 2, 2022 

Local nonprofit A Better Society announced they are partnering with Unite Us, the nation’s leading technology company connecting health and social care, to expand the coordinated care network known as Unite Minnesota. This partnership will help support A Better Society’s community project that addresses the unmet needs of people and families throughout Carver County and nearby communities in Minnesota. The Unite Minnesota network, which launched in the seven-county metro area last spring, is expanding to serve all Minnesota counties by the fall of 2022.

Since 2008, A Better Society has assisted community-based organizations in the Carver County area in kick-starting new ideas and scaling them into sustainable projects. Over a decade ago, A Better Society sought to improve the process of getting people the help they need more efficiently by building collaboration among local support organizations serving Carver County but lacked the required technology. Partnering with Unite Minnesota has brought A Better Society’s vision full circle. 

Unite Us helps connect people to much-needed resources and services such as housing, food, clothing, and household goods, and creates a person-centric ecosystem where clinical and social care providers can ensure that all of an individual or family’s needs are met. 

“Heather and I started A Better Society with one goal in mind – to engage and build community. We are excited to partner with Unite Minnesota’s coordinated care network to ensure no one in our community goes without the services they need. Together, A Better Society and Unite Minnesota will make sure every resident of Carver County has the resources they need to thrive and live healthier lives,” said Khai Tran, Board Chair of A Better Society.

Unite Minnesota securely connects people in need with community-based resources that are difficult to navigate. The network provides a central point of contact where health care providers, social service organizations, and individuals securely access and refer people to needed services while monitoring progress and measuring outcomes.

“Our partnership with A Better Society aligns with our goal to connect residents of Carver County and across the state to critical resources and services they need, regardless of circumstance,” said Allison Alstrin, Community Engagement Manager, Unite Us. “Their work and dedication to community service, government, and healthcare organizations across Carver County make them a natural partner for Unite Minnesota. Our team supports and trains network partners that coordinate care, no matter where in the state people live. Our goal is to increase equitable access to care and services in Carver County and beyond,” she added. 

Khai and Heather Tran, founders of A Better Society, understand the importance of addressing social care to improve overall well-being. They have already connected many local organizations to the network, including Love INC of Carver County, The Humanity Alliance, Waconia United Food Shelf, His House Foundation, Bountiful Basket, Carver County Community Development Agency, Grace Church – Chaska, TreeHouse Chaska, Launch Ministry, St. John’s Lutheran Church & School, and Advocates for Thriving Communities.

“The CDA is excited to be a part of this project. We see this as an excellent opportunity to work more effectively with our partner organizations in making closed-loop referrals,” said CDA, Deputy Director Allison Streich. “We are excited to have a better alternative for the clients we serve by simply making direct referrals through the Unite Us Platform to partner agencies, instead of providing them with another organization’s contact information. We look forward to more agencies joining the network,” she added.
