Heading Home Scott Carver

Meets on the first Wednesday of every month.  We use Teams, but are discussing in person options on a quarterly or 6 month basis.  We discuss the Family Homeless Prevention and Assistance Program, Shelter, Coordinated Entry, and receive updates and info from those on the meeting.  Anyone interested in joining can let Jen know and you’ll be added to the Distribution list.

A sub-committee of Heading Home Scott Carver is the operations call that happens on Thursday mornings at 9:00 am.  This is a brief, 30 minute Microsoft Teams meeting where we run through funding levels for FHPAP, share trends/gaps we see in the community, case consult, etc.  Let Jen know if you’re interested in joining.

Homeless Intervention Team – this was an active group pre-covid. I lead it.  Since COVID I have cancelled the meeting more than I have held it.  I need to find a new time for it and get it back to being in person.  This meeting historically had a presenter from a housing related resource, provided updates on housing resources in general, and left a lot of time for case consultation.  It is a Carver County Specific meeting and the goal is to get all the major HHS departments from the county, the sheriff’s office, public health, and housing agencies together once a month. My goal is to find a new meeting date for June, in person.

Carver County Public Health Advisory Council

The Public Health Advisory Council is comprised of residents from throughout the county who develop and implement a Community Health Improvement Plan (CHIP), which is updated every five years. The plan currently being  implemented is for the period of 2020-2024.  It includes a focus on 4 Priority Issues, with 2-3 goals for each issue.    Action Teams are working to implement the goals, and every member of an action team is part of the Council, which meets 6 times per year to keep progress around the plan moving forward. Meetings are held on the first Thursday, every-other month over lunch at rotating locations in the county.

Development of the five-year CHIP is based upon quantitative and qualitative data gathered in a Comprehensive Community Health Assessment (CHA).  The last CHA was conducted in 2018, so an update CHA will occur in 2023 – resulting in an updated CHIP by the end of 2024 (for 2025-2029).

Communities of Belonging

Communities of Belonging (COB) is a community-by-community initiative, organized by local residents, and supported by the Carver County Public Health Department, as part of the Community Health Improvement Plan.  Currently initiatives have begun in Chaska, Chanhassen, Cologne and Waconia.  The focus is on creating community conditions where everyone is welcomed and feels like they truly belong in their community.  Belonging is essential for individuals to attain their optimal level of physical, mental, and social health and well-being.  This in turn creates healthier communities.

Residents work together to create this type of inclusive community for all of their current residents, as well as all of their future residents, by taking a collective journey together; one which works for their own unique community. It strives to be a community that knows each other, cares for each other, and takes care of each other.  It respectfully welcomes all people at all times; in spite of their differences, and even if they agree on very little.
